Then, include this information on department, county, city, or agency letterhead. Additionally, a PO # must be included on the letterhead. After the request is complete, the authorized purchaser can fax the order to 561-427-0979.
When we receive your agency or department PO, we will call to verify the order. When the order is approved, we will ship your boot order and bill your agency or department. All purchase order balances are due within 30 days.
